I, ________________________ do hereby agree to be spanked by an NMRC member in
public and onstage during the NMRC Warez 2005 Extravaganza at Defcon 13 in
Las Vegas on Friday, July 29 2005. NMRC and DefCon organizers are not
responsible for injury -- physically or emotional -- via the stresses of
public humiliation via corporal punishment. I understand that I may be
photographed and videotaped by the myriad of idiots present, and that said
photographs and videotapes may be posted on all kinds of lame websites,
including NMRC's website, and that I freely allow this.

Additionally I certify I am 18 years of age or older, and that I am bad and
truly need this spanking. I certify that I do not have sharp objects in my ass
and/or ass cheeks, and that I have not been eating at the buffet at the
Flamingo in the past 24 hours. I also agree that if I am too drunk, gross, 
repulsive and/or skanky, no NMRC member may choose to spank me, which I 
understand could be even more humiliating.

I further agree that if I am using the spanking to get back at my significant
other for some perceived wrongdoing, that said significant other is not
